In the various pieces of parchment given to all students at the beginning of the year there are descriptions of the various classes taught at Hogwarts School of Witchcraft and Wizardry. Some were mandatory whilst others were elective. One of those pieces of parchment contained a description for a particular class taught by the witch who had given out the parchment: Transfiguration.
Along with that description of the course there was two other pieces of information attached:
And on the final page a section to be torn off and delivered to the Transfiguration Professor's Office, which also served as the office for the Deputy Headmistress and Head of Ravenclaw House. It was a simple piece of paper for those wishing to enroll.

Name: Zahara Noir Age & School Year: 17 & 7th House: Hufflpuff Reason for choosing Transfiguration: Well I feel I can approve in this field and If I cant well then at least I didn't go down without trying. I also feel this branch of magic is unique and would serve an important purpose at sometime later in life.

Name: Allison Wonderland Age & School Year: 17 and 7th Year House: Hufflepuff Reason for choosing Transfiguration: I'm fascinated by the act of changing one thing into another. Transfiguration is a branch of magic that honestly calls to me in a way no other branch does. It just feels right and I think I can do wonderful things with the skills I'll learn in this class. Maybe an Auror, maybe a professor, or maybe nothing. Regardless it'll be fun to see where Transfiguration leads.
